About the Post-Secondary Data Collection Form

The Post-Secondary Data Collection form collects your academic information from your previous and upcoming terms which allows us to track your academic progress to better support you.

Checklist

Before you begin, you will need the following information and document(s) to complete and submit your form.

Please be sure to have all information ready before you begin.
  1. Your most recent transcript

  2. Your term and cumulative G.P.A.

  3. The number of credits attempted, awarded, and completed (to date)

  4. Details about internships completed in the previous term (if applicable)

  5. Your planned coursework or schedule for the upcoming term

  6. Your Financial Aid Award Letter for the current academic year

  7. Your FAFSA Submission Summary or equivalent for the next academic year
If you plan to transfer schools in a future term, be prepared to provide the following information: 
  1. Information about your new College or University

  2. Your Financial Aid Award Letter for the new school
